Instructions for Customers of the Winners Group

A) Circle the Item that you are purchasing, enter the Number of Units being ordered, multiply the Number of Units times the Total Item Price for that item and enter the figure into the Total Extended Price box for that item. 

B) Repeat the above process for each additional item you wish to order. 

C) Add together all of the figures in the Total Extended Price column and enter the sum on line #1 below. 

D) Calculate any additional freight if the items are to be shipped overseas (for Alaska, Hawaii, Puerto Rico and INTERNATIONAL shipments: add $10.00 for the first item and $4.00 for each additional item) and enter the figure on line #2 .

E) Calculate Texas state sales tax if the items are purchased in Texas and enter the figure on line #3. 

F) Add lines #1, #2 and #3 and enter the sum on the line below labeled Total payment due with order:
